Citizenship or status
U.S. passport, certified birth certificate, or valid permanent resident card.
Photo ID
Government-issued photo ID (state driver's license, state ID, or military ID).
Proof of name change, if any
Marriage certificate, court order, or divorce decree if your documents don't match.
Payment at appointment
Most enrollment providers accept credit, debit, money order, or certified check. Not all accept cash.
